PRACTICAL GUIDE · Marketing

Make clear customer feedback forms and turn responses into action

Good feedback surveys ask a small number of relevant questions, explain how responses will be used and make it easy for customers to reply. This studio creates an embeddable HTML survey with a star rating, recommendation score and optional comments, and lets you analyse responses imported from a CSV file. Actual online response collection requires a form processor you configure. The free tool neither hosts survey submissions nor secretly stores customers’ comments on Silver Ivy servers.

WHAT REALLY MATTERS

Before you start

Useful surveys ask one clear question at a time and tell people how their responses will be used. A short form generally gets better completion than a long, intrusive questionnaire. Decide what action each question will help you take.

How to use this tool

  1. Set a short survey title, brand details, privacy notice and the questions to ask.
  2. Choose an HTTPS form submission endpoint, export the HTML and test the real handler before sharing it with customers.
  3. Import an exported CSV of actual survey responses to calculate average star rating and recommendation score breakdown without uploading data.

A real-world example

A home services company asks customers to rate the completed job out of five, give a recommendation score from zero to ten, and optionally describe what could be better. The owner imports real responses to spot recurring complaints.

Good to know

Treat survey comments and contact details as personal information when they identify a person. Minimise collection, communicate your purpose and store responses securely.

QUALITY CHECKLIST

Check this before you finish

  • Limit required fields and use unbiased question wording.
  • Set up a secure response endpoint and confirm consent language.
  • Test the form confirmation and data handling before promotion.
COMMON MISTAKE

What to avoid

A downloaded survey design does not receive answers until connected to a real system.

WHAT THIS TOOL ACTUALLY DOES

Create a questionnaire and export a form for integration with your chosen response service.

Frequently asked questions

Can I collect responses directly in Silver Ivy?

Not in this free standalone edition. Export the form and configure your own secure submission handler.

What does the recommendation score mean?

A score of 9–10 is typically a promoter, 7–8 passive and 0–6 detractor. A sample percentage is descriptive, not a guarantee of customer loyalty.
